We typically look to faculty listings on websites of universities, colleges, or secondary educational institutions to authenticate you. In the form below, you will see a spot for you to provide an address for a specific web page (not a general school home page) that will help us to determine your eligibility.

NOTE: We do not accept commercial email addresses from providers like yahoo, aol, hotmail, verizon.net, comcast.net, etc., but generally require an institutional (school) email address. We will verify that you are the owner of the email address before we process your application. Once you submit the form below, an email will be sent to you with instructions on how to confirm your address. Only if this step is successfully completed will your application be processed. Processing should occur within three business days. After processing you will either be contacted with an approval message or a request for further information if necessary.
