Permissions Form for the National Center for Case Study Teaching in Science

General guidelines for case study usage are posted at http://ublib.buffalo.edu/libraries/projects/cases/guidelines.html. Usage falls into two catagories, standard and non-standard. Standard usage does not require formal permission. Non-standard usage does require formal permission, and the form below can be used to make such a request. We offer the following licensing options:
